In submitting my Testimonial, I hereby agree as follows:

1. I hereby give Direct4Effect LLC, dba The 6-Week Extreme Life Makeover (“the Company”), or its successor organization(s) or assigns, the irrevocable right to use this testimonial and questionnaire responses for marketing and advertising purposes. I realize my testimonial may be subject to minor editing, abridgment, etc. for advertising copy purposes, however it may not be changed in any way that would materially affect the gist of my overall portrayal of this service.

3. I hereby transfer my right of copyright of this testimonial to "the Company", or its successor organization(s) or assigns.

4. I certify that I am of legal age and competent to agree to this release. I agree that this release shall be binding on me, my legal representatives, heirs, and assigns. I certify that I have read this release and am fully familiar with its contents.

5. I hereby grant permission to the Company to use the following as part of my testimonial:

6. I agree that no advertisement or other material need be submitted to me for approval and the licensed parties shall be without liability to me for any distortion or illusionary effect or other adverse result arising from the publication of my picture, portrait or likeness.

7. I warrant and represent that this license does not in any way conflict with any existing commitment on my part. I have not heretofore authorized (which authority is still in effect), nor will I authorize or permit, the use of my name, picture, portrait, likeness or testimonial statement in connection with the advertising or promotion of any product or service competitive to or incompatible with the Company.

By submitting your Testimonial, you agree: (1) that you have read the terms stated above; (2) you understand the terms stated above; (3) you intend to form a legally binding contract by entering your name and clicking on the "Submit" button; (4) a printout of the terms stated above will constitute a "writing" under any applicable law or regulation without regard to when or by which party the printout is made; and (5) you agree to abide by all the terms of the Agreement stated above.
